							<p>This type of medication paralyzes the muscles. More specifically, it prevents the <b>neurotransmitter acetylcholine</b> from being released from one neuron to activate the next. It is like putting a piece of plastic between your plug and electrical socket – nothing gets through. If you are into medical science, here is a good article to read: <em><b>Walker TJ, Dayan SH. Comparison and overview of currently available neurotoxins. J Clin Aesthet Dermatol</b>. 2014;7(2):31-39.</em></p><p>Right now, the most common BTX-A products are the “Big 4”. <b>Botox, Dysport, Jeuveau, and Xeomin</b>. Each of these is slightly different from one another and is used in slightly different ways. All have their niches and their specific uses. Some work quickly, others take time. Some cover larger areas, some for smaller areas. There is even an alternative one if you are allergic to the other BTX-A agents. Each practitioner uses them in a different way. Misinformation is much more prevalent than good information. Politics are everywhere. Experts who work as underwater basket weavers have much more experience than seasoned physicians. Much of the medical information out there has been corrupted by bad information and politics. How do people get through that? </span><b>What do you do?</b></p><p><b>Listen to people who care and work at giving good information.</b></p><p><span style="font-weight: 400;">First, let us start with some disclaimers. Medicine is an art. Doctors preach science. Physicians take science and apply it to life. There are no absolutes in medicine. We take what we think is right, and present it in a balanced, educational way.</span><b> I am going to attempt to do that here.</b></p><p><span style="font-weight: 400;">Second, it is absolutely okay to disagree with me. This is my interpretation of the data and information that we have up to this point. The information is constantly changing. Over time, the information we thought was correct might turn out to be wrong. Some research is manipulated to say one thing, rather than another. This is where judgment, wisdom, and education come in. One of my favorite sayings is that my medical degree beets your internet degree. I don’t know how long you have been alive, but I went through this with a </span><b>novel new virus called</b> <b>HIV</b><span style="font-weight: 400;"> a few decades ago.</span></p>						</div>
				</div>

							<p><span style="font-weight: 400;">The research is showing that people who have a non-robust immune system start to lose their resistance after </span><b>about 8 months of being vaccinated.</b><span style="font-weight: 400;"> This is especially true for people with moderate to severe immunocompromise – </span><b>like diabetics, people with immune disorders, or cancer patients.</b><span style="font-weight: 400;"> Not only are the immunocompromised affected by this, but the research is also starting to point toward healthy older people and people with other medical conditions. These individuals do not completely lose their immune response, it just weakens them so the virus can make you sicker the next time it happens.</span></p><p><span style="font-weight: 400;">In addition, there are new strains of the virus popping up. This was expected. The proteins on the virus change to try to get away from the immune response. Again, expected. </span><b>This is just like the Flu and how we need to take a vaccine every year to fight it.</b></p><p><span style="font-weight: 400;">Another point is that a vaccine does not prevent you from getting sick. It keeps you from getting really sick and possibly preventing severe side effects or even death. If you have been vaccinated, have a strong immune system, and get the CoViD virus – </span><b>you will still get sick,</b><span style="font-weight: 400;"> but just not so sick you are stuck in the hospital, or worse. Also, being vaccinated means that if you get sick again, you will not spread the virus as easily yourself or not spread it at all.</span></p><p><span style="font-weight: 400;">Notice, in all of this information, there is a good deal of variability. Nothing is absolute in medicine. We try to cover 98% of people, but there are always extreme cases.</span></p>						</div>
				</div>

							<p><span style="font-weight: 400;">Okay, so we have an immune response that putters out, a virus that changes, and everyone responds a little differently. </span><b>Think of the flu, people!</b><span style="font-weight: 400;"> We will have a yearly vaccine, or booster if you will, based on what is out there. This might also happen when our immune system starts to unwind because we are not seeing the virus enough.</span></p><p><span style="font-weight: 400;">Now, some good news. There is research and testing on a </span><b>new vaccine that will give long-lasting immunity</b><span style="font-weight: 400;">. I don’t want to say permanent, but long-lasting. We will see where that goes.</span></p><p><span style="font-weight: 400;">These are the basic facts we are presented with. Here is what I am suggesting to our patients:</span></p><p><span style="font-weight: 400;">If you have any medical conditions or are over the age of 50, </span><b>I suggest a booster.</b><span style="font-weight: 400;"> We are using the J&amp;J Vaccine as the booster because it is the best match to the new variants coming out. If you want a Maderna or Pfizer, that is fine too.</span></p><p><span style="font-weight: 400;">You need to wait about </span><b>6 to 8 months since your last injection.</b><span style="font-weight: 400;"> Otherwise, it overlaps, does not help as much, and makes you feel horrible.</span></p>						</div>
				</div>

							<p><span style="font-weight: 400;">Unfortunately, unless you are a more senior physician, know the politics and how research can be manipulated, </span><b>it is very difficult</b><span style="font-weight: 400;">. Everyone is trying to push an idea, or agenda, or way of life right now. Take what you read with a grain of salt. Absolutes do not exist, other than death and taxes.</span></p><p><span style="font-weight: 400;">Try to use good, reputable websites. Remember; </span><b>Google, Facebook, Twitter, and many other platforms are owned companies</b><span style="font-weight: 400;"> that may have a political agenda. Unfortunately, even information from trusted sources could be not-so-trusted due to the political environment. Ask people in the know who are not jaded and are looking at all sides of the argument.</span></p><p><b>We will do the best we can to get through this together.</b></p>						</div>

							<p><strong>1)</strong> Passport or Federal acceptable ID (Like a driver’s license with a Real ID star);</p><p><strong>2)</strong> Their immunization records, or we will substitute for that (see below)*;</p><p><strong>3)</strong> Their address, home and cell number (can be the same), email;</p><p><strong>4)</strong> marriage certificate or name change certificate if this has happened since the last filing;</p><p><strong>5)</strong> A letter from their primary care if they have chronic medical conditions. Ie: Hypertension, diabetes, etc, noting they are under routine care and compliant with treatment. This includes any psychological conditions too, such as anxiety or depression.<u></u></p>						</div>

							<p>*If no immunization records, we have to test or boost – based on what is least expensive:</p>						</div>

							<p><strong>If you have insurance, and your own doctor</strong> – best to get it done before you come in. It can save you a lot of money.</p><p>There have been some changes over the last year or so from the Federal Government for immunizations. <strong>Varicella</strong> (Chicken Pox), and the <strong>MMR</strong> (measles, Mumps, Rubella). If you do not have your records, a booster series is initiated. Normally 2 shots about 1 month apart. You only need the first shot to have the forms signed off on. The applicant pays the pharmacy or health department directly, not us. This saves you money.</p><p>For the TDaP and Influenza, <strong>a single injection is needed</strong> – you will pay the pharmacy or health department directly.</p><p>For children,<strong> it is a lot more complicated.</strong> It is best to have your pediatrician have the child reviewed for immunizations.</p><p>Anyone who wants to know what immunizations they need can go to the following website:</p><p><u>https://www.cdc.gov/immigrantrefugeehealth/exams/ti/civil/vaccination-civil-technical-instructions.html</u></p><p>and the chart:</p><p><u>https://www.cdc.gov/immigrantrefugeehealth/exams/ti/civil/vaccination-civil-technical-instructions.html#tbl1</u></p>						</div>

							<p><strong>1) Standard Labs:</strong> test for STDs and important public health diseases. These are mandatory and set by the Federal Government. This goes to the lab.</p><p><strong>2) Special Testing:</strong> This gets added on for several reasons. Mostly due to a random selection or a special evaluation request. It can be a drug urine screen, or Hemoglobin A1C for a diabetic if not up to date, Ect. The patient pays our cost, which is way less than paying at the lab. But you are welcome to pay at the lab directly too.</p>						</div>
				</div>

							<p>The CDC has released new guidelines for wearing masks pre and post-vaccination. <strong>Let’s keep it simple.</strong></p><p>If you have completed your vaccine series <strong>two weeks ago</strong>: you may go without a mask. As long as you are in a relatively low-risk environment, you may go au naturale without the mask. If you are working in the CoViD ward, you may still want to wear one. Also, if you intend to lick anyone who currently has CoViD, the mask will also prevent this!</p><p>If you have taken your first Pfizer or Moderna Vaccine, or it has been<strong> less than two weeks</strong> after your second shot &#8211; mask up baby! You might be covered but with this bug, it&#8217;s better safe than sorry.</p><p>If you have taken the Johnson &amp; Johnson (J&amp;J) vaccine &#8211; <strong>give it 6 weeks after your shot.</strong> It should not take that long, but it&#8217;s better safe than sorry. For some people, it will take much longer for their immune systems to adapt and fully mature.</p>						</div>

							<p>If you have had CoViD, this becomes a bit tricky. Here is what we are doing in my practice: <strong>We test for immunity.</strong> If the person has IgG antibodies, it is the same as being immunized. <strong>No need for a mask!</strong></p><p>What about a vaccine for those who have had CoViD? Just one shot, thank you. You will only need a single booster of<strong> any of the three available vaccines.</strong> Fair warning &#8211; you are going to feel like you have a viral infection for a day or two after the shot. You are not sick, it is just your immune system getting worked up over the vaccine &#8211; and that is a good thing.</p><p>This is just a personal guideline and this is what we are telling our patients at our office. We practice personalized medicine, so each person could be a little different. Only check with your doctor. Remember, most medical degrees will trump Google / Twitter / Internet / Friend degrees&#8230;</p><p>And get ready for those funny looks from the Mask Police! When asked, just start singing “A Change Would Do You Good”.</p>						</div>

							<p>There are new CoViD variants being discovered weekly now. Do not be alarmed or surprised. <strong>It is very normal for this type of virus to shuffle it’s genes to try to improve.</strong> Sometimes it works, sometimes it does not. There most likely have been many variants that have come, gone, and are not discovered. What is more surprising is that the experts have been caught off guard by this. The cCorna Viruses have been known to do this for decades.</p><p>There are variants now that escape (or are not covered by) the current Pfiser and Moderna vaccine. That is because these two vaccines attack a single spot on the virus surface that has changed. The multitarget vaccines (some attack 5 or more places) are covering the changes. <strong>These vaccines will be in the market soon.</strong></p><p>In the long run, we will be handling this just like the flu. Every year, the medical experts will be trying to figue out what cocktail of targets to put in the vaccine to try to match up for what is coming. Stay tuned&#8230;</p>						</div>

							<p>I prefer to say there is something that pisses off and wakes up your immune system, and then parts of the virus for your body to make antibodies against.</p><p>The immune system <strong>only makes antibodies when it feels that the infection or threat is significant enough</strong> that a bunch of energy needs to be spent. That this what #2 does. It turns on the immune system enough to wake it up. Kind of like poking a sleeping bear with a big stick.</p><p>Once awake, that bear is going to be looking for something that woke it up. That is when those foreign virus parts will be picked up; <strong>then wham-o &#8211; immunity with antibodies.</strong></p>						</div>

First and foremost &#8211; <strong>ignore people speaking as experts who really are not.</strong> Wash those paws, keep it clean, don’t lick things you shouldn’t be. Masking helps. Fresh air helps. Breathing stale air with lots of people you don’t know is bad Juju. Not washing your hands before you eat or touch your face is also not a good idea.</p><p><strong>Expect this to roll into a yearly vaccine.</strong> Expect some people to try to make noise about what is bad and how everyone needs to listen to them. In other words, expect hysteria and people to speak without fully understanding and thinking. Then ignore them. Focus on the current, future, and positive. That does not include the United Kingdom (UK) Variant that is in the US now.</p><p><strong>2)</strong> Why are there Variants, and what does it mean. New variants come about by the virus evolving. There are changes in proteins. Some work and make the virus better. Some don’t, and the virus does not survive. This is part of nature and the evolution of viruses. The new variants are either as virulent or more so than the original. The new variants are more &#8220;sticky&#8221; to the receptor, and for a number of reasons, more virulent.</p><p><strong>3)</strong> The vaccine, does it still work? The vaccines we currently are using in the US (Pfizer and Moderna) both are directed at this spike protein and still cover the variants, just not as well. Yes. We do the same thing with the flu vaccine every year. Some of the vaccines are easily modifiable and can be changed quickly to keep up. Some will be more difficult to change. 							<p>CoViD-19: Corona Virus  Disease 19. First discovered in the city of Wuhan, China, caused by a unique (new) virus. We consider this location the epicenter of the disease.<br /><br /><strong>SARS-Cov-2:</strong> Severe acute respiratory syndrome &#8211; Coronavirus &#8211; 2.  The official name for the Virus discovered in Wuhan, China.<br /><br /><strong>Vaccine:</strong> Something put into your body to cause an immune response. The idea is that if your immune system has seen something before, it can quickly get involved when you see the same thing again. The faster your immune system kicks in, the faster you clear the disease &#8211; and the less sick you get.<br /><br /><strong>Hapten:</strong> This is what the antibodies attack.<br /><br /><strong>Virulence:</strong> How many infectious particles does it take to get you sick.<br /><br /><strong>Variant:</strong> In infectious disease, it is an infectious particle (virus, bacteria, ect.) that is basically the same as the original, but changes the covering around the particle. This is done as part of evolution. To become more infectious, get away from the immune system, grow better in different environments, etc.</p>						</div>

							<p>The new variant in the United Kingdom is not a new virus. Simply, there was a single change in one of the proteins on the outside lining of the virus. This is not the first variant to emerge, but may be the first important one. Now, this can be a very big deal, or not a big deal at all. With this info being so new, we are trying to figure this out. Right now, it looks like it is a mixed bag.<br /><br />All the vaccines have one thing in common, they all get the immune system to attack the virus at a specific point. This target, is a protein on the membrane of the virus. Think of it as a specific type of tree sticking out of the ground.  Change the leaves on the tree, and it looks different.  If it looks like a Pin Oak, they attach. If it looks like a White Pine, they leave it alone. This is the virus’s hapten.<br /><br />This variant changed the tree. Not enough to fool the vaccine. The vaccine, and your antibodies, will still see this virus. Your body will still see this as the same SARS-CoV-2 virus that needs to be cleared. Good news!<br /><br />Now the not-so-good-news. The change made these trees attach to your cells a lot better. Think about it as sticking to your cells so the virus can get in. The ACE-2 Receptor is where this virus attaches, then gets pulled into your cells. This new variant really holds on a great deal better, and can get into the cell easier and in larger numbers.<br /><br />This is part of virulence. So people whom are already at risk. Those are people whom have Diabetes, are older than 60, have multiple medical conditions, etc; all have increased numbers of these ACE-2 Receptors. They need lower number of viruses to become very sick.<br /><br />Unfortunately, everything is too new to assume anything else on what this variant will do. But as a comparison, this is what the Influenza A and B virus do each year.</p>						</div>

							<p>Every person must have the following for the appointment:</p><p><strong>1)</strong> Passport or Federal acceptable ID (Like a driver’s license with a Real ID star);</p><p><strong>2)</strong> Their immunization records, or we will substitute for that (see below)*;</p><p><strong>3)</strong> Their address, home and cell number (can be the same), email;</p><p><strong>4)</strong> marriage certificate or name change certificate if this has happened since last filing;</p><p><strong>5)</strong> A letter from their primary care if they have chronic medical conditions. Ie: Hypertension, diabetes, etc, noting they are under routine care and compliant with treatment. This includes any psychological conditions too, such as anxiety or depression.</p>						</div>

							<p><strong>USCIS Requirements:</strong> If the applicant cannot read, speak, and write English, they must have an interpreter with them for the initial visit with the doctor, and must return at signing to sign the forms.</p>						</div>

							<p>If you have insurance, and your own doctor – best to get it done before you come in. It can save you a lot of money.</p><p>For <strong>Varicella</strong> (Chicken Pox), and the <strong>MMR</strong> (measles, Mumps, Rubella), we normally test. There is a draw fee set by the lab. If these tests say immune, we are done. If they say not immune, a booster series is initiated. Normally 2 shots about 1 month apart. If the test says low, but not immune, then only 1 booster is needed. The boosters are about the same cost of testing, maybe a little more. The applicant pays the pharmacy or health department directly, not us. This saves you money.</p><p>For the <strong>TDaP</strong> and <strong>Influenza</strong>, it is much less expensive and harmful to get the immunization – so they go to the pharmacy for that, and pay the pharmacy or health department directly.</p><p>For children, it is a lot more complicated. It can be a drug urine scree, or Hemoglobin A1C for a diabetic if not up to date, Ect. Patient pays our cost, which is way less than paying at the lab. 							<p>Accurate scientific information regarding this COVID virus is a political hot mess in the US, so it is difficult for people to understand or believe what they are reading and hearing from politicians and medical experts.</p><p>Here, to the best of our knowledge today (we will adjust if that changes), are the<strong> FACTS</strong>:</p><p>This <strong>SARS2 Virus</strong> started as a small local outbreak in the industrial city of Wuhan, China sometime in the <strong>November-December 2019</strong> time-frame. The socially acceptable name for this virus is <strong>SARS-CoV2</strong>. The virus hopped to humans in Wuhan after passing through a series of other animals – we may never know which ones – and the only reason we even care is that we would like to put a lid on that source.</p>						</div>

							<p>The US has invested <strong>billions of dollars</strong> researching and trying to understand this disease. To date (late June 2020) there have been <strong>28,000 papers published in the six months</strong> since this disease emerged – and we are still learning as we go! Right now, here is what we think the facts are:</p><p><strong>A)</strong> The virus enters the body via <strong>the mucous membranes</strong> (wet surfaces) of your upper and lower airways (nose, mouth, and lungs), your digestive tract, and your eyes. Like HIV, it can <strong>probably</strong> also enter through your vagina and rectum during some types of sex. VERY importantly, this virus cannot enter your body through intact skin, so just touching it with your hands or body is not dangerous – unless you then touch any of the above mucous membranes. <strong>Don’t touch your face</strong> until you have washed your hands very well!</p><p><strong>B)</strong> Based on the number of viruses that first get into you (your viral exposure load) – and your personal ability to fight the virus that particular day (which changes) &#8211; it takes about<strong> 3-5 days</strong> for our high sensitivity nasopharyngeal swab <strong>‘PCR</strong>’ test should show positive. This means you have the virus growing (replicating) in you.</p><p><strong>C)</strong> About <strong>7-15 days</strong> after the virus begins to replicate – the timing is based on <strong>YOUR PERSONAL</strong> immune system and how the battle goes &#8211; your white blood cells will slowly begin making its first <strong>IgM antibodies</strong>, and our IgM blood test (serology) will begin to turn positive.</p><p><strong>D)</strong> About <strong>7 days after that</strong>, if still needed, your white blood cells will <strong>SWITCH</strong> to making the more powerful and virus-specific <strong>IgG type</strong> of antibodies, and our IgG blood test will slowly begin to turn positive.</p><p><strong>E)</strong> While you are recovering from COVID, your <strong>IgM levels</strong> (and the cells making it) will peak and then very rapidly disappear, followed by a an even bigger IgG peak – which will then gradually drop over the next <strong>3 months to 30 years</strong> – depending on YOUR individual immune system and how severe the COVID affected YOU.</p><p><strong>F)</strong> This is your IgG immunity. There is another important part of this story, concerning other white blood cells (immune cells) in your body. These cells fought the virus and cleaned up the battlefield afterward, and some of them go into a ‘veteran’s retirement home’ where they will remember how they fought the COVID Battle of 2020. <br /><br />The more of these memory cells that you make, and the better their retirement home conditions, the better and longer will be your long-term immunity to the virus – a flash-in-the-pan-catch-it again-next-year cure or a semi-permanent immunity.</p>						</div>

							<p><strong>1.) Virus swab +, IgM/IgG negative:</strong> You are infected and can be infectious to other people around you.</p><p><strong>2.) Virus swab +, IgM +, IgG negative:</strong> You are infected, you are likely getting better, but you ARE infectious to other people.</p><p><strong>3.) Virus swab +, IgM +, IgG +:</strong> You are infected, you are VERY likely infectious to other people, but you are well on your way to getting over this.</p><p><strong>4.) Virus swab -, IgM +, IgG +:</strong> You are clearing, or have cleared, the VIRUS, you <u>probably</u> aren’t infectious from your respiratory system. <strong>CAUTION!:</strong> However, if the virus infected your kidneys, guts or testes – you might still be putting out (shedding) virus that could infect others, so you need to practice safe behaviors for at least 6 weeks – the longest time MOST shedders have been tracked. There is always the oddball who sheds longer, it is best to stay on the side of caution as you get better.</p><p>Virus swabs and blood PCR tests are negative, IgM test is negative, serology tests are positive for strongly virus-killing (neutralizing) IgG antibodies: You are well into your COVID recovery phase, you have good immunity now. You are <strong>PROBABLY</strong> not infectious, but see the <strong>CAUTION</strong> above.</p>						</div>

							<p>This means that if you see this exact virus again, your body will be ready for it &#8211; and you will have <strong>little or no outwards symptoms</strong> from your next encounter. For now, we believe most mild-to-severe COVID survivors will easily clear any near-future CoViD-19 infection with relative ease, <strong>without having noticeable symptoms</strong>. If you have only weak symptoms or are entirely asymptomatic, your risk of a future Covid RE-infection is <strong>much higher</strong>. <br /><br /><strong>How high?</strong> – We don’t know yet. What if the virus changes, will I still be immune? We don’t know yet, but for sure it will be a person-specific, virus-change-specific answer when we get it. So, yes, there is immunity. We are getting this put in a logical way to present it, and will post / send it out as soon as I have the info from the CoViD committee.</p>						</div>

							<p>As of 9/19/2020, in Horry county, there is on average of <strong>32 new cases a day</strong>, which has dropped significantly.</p><p>For people whom have symptoms of CoViD and are being testing, t<strong>he positive loads are about 8.4%</strong>. That means if someone is sick with CoViD symptoms, about only <strong>1 in 11 actually have the disease.</strong></p><p>Unfortunately, there have been <strong>195 deaths</strong> in our county as of 9/19/2020, where CoViD is involved. That does not mean they died from the disease, simply they had CoViD some time during their illness, and the patient passed. These are mostly <strong>much older adults with pre-existing medical conditions</strong>. The average age is <strong>74</strong> but someone with multiple medical conditions whom was <strong>25 year old</strong> also passed.</p><p>Compared to other states, our area and South Carolina has one of the highest cumulative rates in the country. Again, our number are skewed by visitors being tested, but this is an important distinction. Many of our residents have had the disease, and are now over the infection. Most of which will form some type of immunity.</p><p>Our goal is to keep as many people from dying as possible so we can get a vaccine out to the population.</p><p><strong>Washing your hands, don’t touch your face, etc:</strong> The most important thing you can do. Virus cannot get through intact skin. Keep it clean babies!</p>						</div>

The feeling is that your chances of being infected will go down if you have a lower “<strong>Viral Load Exposure</strong>”</p>						</div>

							<p>In Horry county, a little over <strong>2% of the population</strong> has either been or is currently infected with the disease as estimated by the DHEC. On average, there are <strong>100 new cases daily</strong> with all the testing we are doing.</p><p>For people who have symptoms of COVID-19 and are being tested, we are seeing about <strong>18% positive results</strong>. That means if someone is sick with COVID-19 symptoms, there is a chance that only <strong>1 in 5 of these people</strong> actually have the disease. Right now, it appears to be <strong>0.3%</strong> of the population or <strong>3 in 1000</strong> people have the disease.</p><p>Unfortunately, there have been <strong>186 deaths</strong> in our county as of September 2nd, 2020 as a result of COVID-19. This does not mean that they died from the disease, only that they had COVID-19 at some point during their illness and the patient had passed on. In most of these cases, these were much older adults with pre-existing medical conditions, roughly around the average age of 74-years-old. In a much rarer case, there was a 25-year-old with multiple medical conditions who unfortunately died due to complications with COVID-19.</p>						</div>
